Change your name and civil status (include updated contact information like address and landline numbers) for the following:

  1. Government identification cards / documents
    • Passport
    • SSS/GSIS status
    • BIR/TIN (Tax Identification Number)
    • Philhealth
    • Pag-ibig
    • Driver's License
    • NBI
    • Postal ID
  2. Personal/Financial Identification
    • Insurance
    • Company ID
    • Health cards (e.g., Philamcare, Maxicare, etc.)
    • Bank accounts / ATM cards
    • Credit cards
  3. Professional Affiliations
    • License (e.g., PRC, etc.)
    • Organizations

Don't worry. The list looks overwhelming, but managing them bit by bit accomplishes the job. Most of these can be updated through the help of your company HR personnel. They can provide you with the forms (why, your HR might have their own checklist for your perusal!) and can even file them for you. If not, carefully plan which ones are your priority so that you can schedule your leaves accordingly. Browse the internet for the requirements so that you are equipped with the documents when you go to each and every office for the updates.
